Step 1

How You’ll Connect with Us

We welcome two categories of new patients:

1. In-Person Visits

If you’re planning a visit to our flagship location in Darien, Connecticut, we will:
  • Review your insurance eligibility
  • Bill directly if we are in-network
  • Assist with understanding out-of-state or out-of-network benefits

2. Telehealth Consultations

For international and national patients unable to visit initially:
  • Comprehensive video consultations with Dr. Nassiri
  • Self-pay basis
  • All procedures will ultimately require in-person care

Step 2

Submit a New Patient Registration Form

Regardless of visit type, all patients begin by:
  • Completing the intake form on our website
  • Uploading relevant records (imaging, prior consultations, referrals, insurance, clinical photos)
  • Submitting insurance info (even for self-pay patients)
Our care coordination team will promptly review your case and respond.

  • A telehealth video consultation

TVCG CARES is a global destination for the most challenging cases of:
  • Vascular malformations, including low-flow capillary, venous, and lymphatic malformations; high-flow arteriovenous malformations; and mixed varieties thereof
  • Hemangiomas, both infantile and congenital
  • Mixed and syndromic vascular conditions, including but not limited to: Klippel-Trenaunay Syndrome, CLOVES Syndrome, PIK3CA-Related Overgrowth Syndromes (PROS), Proteus Syndrome, Maffucci Syndrome, etc.
